A survey is being conducted as part of a faculty sabbatical project. The survey, which can be accessed at this link, focuses on accessibility of health care for UFV students/employees and seeks to determine whether medical services on-campus are needed and would be utilized. This survey should take no more than 10 minutes to complete and will inform a recommendation for how UFV could move forward to provide health care services for students and employees in the future.
Your participation is voluntary and anonymous. All surveys submitted will be utilized in the data analysis. Please answer any/all of the questions that you are comfortable answering.
