Job Summary:

The Seniors Community Connector will work with seniors who are referred from medical practitioners to connect with community supports and services through referrals, applications, advocacy, and introductions. Additionally, the Seniors Community Connector will host and/or facilitate educational training for seniors and community providers.
The Seniors Community Connector will work in collaboration with the Seniors Services team to create an environment where seniors at-risk of frailty will be able to access resources and support.

Qualifications:

Education, Training and Experience:

  • Diploma in Social Service or related human services field.
  • Strong knowledge of seniors’ issues and challenges related to healthy aging.
  • Experience working with seniors and diverse populations from different cultures and socio-economic backgrounds.
  • Knowledge of the community resources, programs and services.
  • Minimum two (2) years recent related experience.
  • Valid driver’s license and reliable vehicle with appropriate insurance.

  • Demonstrated ability to connect with and support senior clients.
  • Demonstrated skills in the areas of crisis intervention and conflict resolution.
  • Strong collaborative skills and proven ability to establish and maintain effective working relationships with all internal and external contacts.
  • Excellent written and oral communication skills and ability to clearly explain instruction to others.
  • Ability to work independently with strong time management and organizational skills.
  • Strong computer skills.

Key Duties and Responsibilities:

  1. Maintains an active caseload of seniors with short-term needs through a referral stream from medical practitioners and community agencies.
  2. Connects, liaises, and establishes partnerships with local medical practitioners to establish and maintain referral stream.
  3. Completes intake process to assess client strengths, needs, abilities, and risks by interviewing and observing behaviour. Use various assessment tools to determine challenges, needs and risks. Develops a service plan with the client aligned with contractual obligations, policy, and program guidelines.
  4. Provides support and guidance to clients to address issues related to healthy aging in an individual setting. Use skills such as active listening, conflict resolution, development of routines and structures, and resource referrals and access to support the client to resolve issues, meet their needs and decrease their risk of frailty.
  5. Facilitates, or organizes facilitators to, provide educational training for seniors and service providers, on topics such as healthy aging and other relevant topics.
  6. Participate in collaboration with community agencies through networking and attending the Healthier Seniors Task Team.
  7. Supports clients to access community resources.
  8. To recruit, screen, retain, train and supervise volunteers.
  9. To match volunteers with seniors and the services they need. Monitor the uptake and needs of seniors using/wanting the service.
  10. To participate in the Seniors Community Connector Community of Practice.
  11. Effectively collaborate within the Agency’s and community’s Seniors Services to provide multidisciplinary care for the best interest of the client.
  12. Completes all client case notes and reports, and other administrative duties as required.
  13. Documents and evaluates progress of work through regular reports to the funder and Director.
  14. Maintain a high level of confidentiality in all matters related to clients and community partners.
  15. Complete all mandatory learning requirements.
  16. Performs other relevant duties as may be required.

General:

  •  Wage rate as per BCGEU Collective Agreement , Program Coordinator 1 –Grid 12 – $29.17
  • This position requires BCGEU membership.
  • This position is open to all persons.
  • This position requires successful completion a criminal record check.
  • This position will report to the Senior Services Coordinator.
